Student Enrollment
To register your child at the school, you will need the following:
- Birth Document (birth certificate, Baptismal certificate, passport)
- Immunization Record (See below)
- Proof of Residence (See Below)
- IEP, if applicable
- Parent/Child must be present
- Parent must provide picture ID
- Affidavit of parental/Legal Guardian Identification
PLEASE BRING ORIGINALS OF ALL YOUR DOCUMENTS. We will make our own copies and return the originals to you.
Parent/Legal Guardian must present VALID identification and Proof of Residence (see below) to pick-up an enrollment packet.
Proof of Residence:
A school may accept one or more of several types of documents from the parent/legal guardian as reasonable evidence that the pupil meets residency requirements for school attendance in LAUSD.
Examples:
- Utility service bills (gas, water, electric)
- Property Taxes, rental or lease agreement
- Official government mail (Cal Works, Social Security)
LAUSD does not accept telephone bills or driver's licenses as proof of residence. ALL documentation must be current and must identify the parent or legal guardian as the resident.
